Use the following evaluation forms (PDF format) to assess the Risk Watch program in your community. The forms include: "Caregiver Questionnaire" for preschool-grade eight, the "Teacher Survey," "Champion Report Form" and "Risk Watch Test Scores Summary Sheet." Forms A-E and the "Risk Watch Test Scores Summary Sheet" should be completed by your Risk Watch teachers and returned to the designated Champion. The information that you gather will help you improve and expand your education efforts.
